About this questionnaire
This intake questionnaire helps us evaluate the child's daily physiological stress, sleep integrity, metabolic balance, and endocrine function.
Patient Details (Optional for Print):
Include Section D: Endocrine & Hormonal Fluctuations?
What is Section D? This section focuses on physiological and hormonal changes specific to adolescents, particularly females. It evaluates the impact of menstrual cycles, cyclic pain, mood swings, fluid retention, and inflammatory teenage acne.
Should I include it? Select 'Yes' if the patient is a teenager (especially female) experiencing hormonal development or menstrual cycles. Select 'No' if the patient is a pre-pubescent child or if these specific cyclic hormonal questions do not apply.
